Registering as a Patient at West Coker Surgery.

If you wish to register as a patient here at West Coker and you live within the practice boundary, please call in to see us and collect a registration pack.

When you register you will be asked to fill out a Registration Form (GMS1) - you can access an online version of this to print off if you prefer (see below) and bring in to the surgery.

In addition to the GMS1 form, we also require you to complete a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.  This form is not available online; you will need to come in to the surgery to complete it - along with other forms regarding use of contact details (text reminders etc.), and an application form for you to access your patient notes electronically. 

We can not register you as a patient until you have come in to the surgery to complete all the paperwork.    

We provide GP services in line with the national GP General Medical Services Contract.  We also provide some additional services which may be different from from your present practice or other GP practices in the area because these additional services are not mandatory but vary from practice to practice, depending upon their size, staff, training and workload.  Before you decide to register at West Coker, we strongly advise you to read both the practice leaflet and the practice leaflet supplement to ensure that the services we provide suit your needs.
